Certified - Position - SPEECH AND LANGUAGE PATHOLOGY

  2. Georgia Teacher Annual Evaluation Summary Report (If you have previously taught in Georgia please use the Georgia Form. If not, please present other evaluation instruments)
  3. Copy of all test scores (TCT, Praxis I, Praxis II, GACE Basic Skills and GACE Content scores or out-of-state scores)
    (If you are exempt from taking PRAXIS, please supply copies of exempting scores.)
  4. Copy of current teaching certificate(s) - Georgia or out-of-state
ADMINISTRATIVE:
  1. Official transcripts from each college attended

  3. Copy of valid Leadership Certificate for the position you are seeking
CLASSIFIED:
  1. Copy of High School diploma or official transcripts from each college attended, if applicable
  2. Copy of valid driver's license, if applicable
SUBSTITUTES: Please attach with your application the documents below that pertain to you.
  1. Copy of degree or transcript from highest level of education from an accredited institution (MINIMUM REQUIREMENT of 60 earned semester hours or 90 earned quarter hours)
  2. Copy of current teaching certificate for Georgia or Out-of-state

  4. Copy of Georgia Teacher Annual Evaluation Summary report
